Day One
Introduction to accident investigation

  • Definition of incidents and accidents
  • Relationship between incidents and accidents
  • Human contribution to accidents
  • Understanding Near Misses
  • Near Miss case studies
  • Avoiding ‘blame culture’
  • Accident trend analysis
  • Accident reporting 
  • Use of accident incidence and frequency rates
  • Incident and accident reporting, forms and software

Day Two

Accident investigation

  • The incident investigation process
  • Gathering information – witnesses, photographs, videos, samples, site plans etc.
  • Accident investigation
  • Witness interviews – technique and questions
  • Analysis of information – Immediate and Root causes (the 5 ‘Whys’)
  • Use of Fault Tree Analysis (FTA)
  • Identifying suitable control measures to prevent incidents and accidents
  • Developing an action plan to implement control measures
  • Assessing the success of accident prevention measures
