- Use of accident incidence and frequency rates
- Incident and accident reporting, forms and software
Day Two
Accident investigation
- The incident investigation process
- Gathering information – witnesses, photographs, videos, samples, site plans etc.
- Accident investigation
- Witness interviews – technique and questions
- Analysis of information – Immediate and Root causes (the 5 ‘Whys’)
- Use of Fault Tree Analysis (FTA)
- Identifying suitable control measures to prevent incidents and accidents
- Developing an action plan to implement control measures
- Assessing
the success of accident prevention measures
